Plan contents:

This project period can use my country's years of experience in the health management mechanism of metabolic chronic diseases to help improve its chronic disease prevention and treatment system and enhance the self-care ability of chronic disease patients and high-risk groups in China, thereby reducing national medical resources and financial burden. content include:

1. Strengthen the promotion of strategies related to the prevention and treatment of metabolic chronic diseases.
2. Strengthen the ability of medical institutions to prevent and treat metabolic chronic diseases.
3. Improve community people’s self-management capabilities required for the prevention and treatment of metabolic chronic diseases.

Project goals:

The overall goals of this project are as follows:
1. Strengthen the promotion of strategies related to the prevention and treatment of metabolic chronic diseases
(1) Policy planners visit Taiwan.
(2) Provide a report on promotion recommendations based on the country’s existing metabolic chronic disease strategies and plans.
(3) Organize at least 1 international symposium on "metabolic chronic diseases".

2. Strengthen the ability of medical institutions to prevent and treat metabolic chronic diseases
(1) Strengthen the professional capabilities and case management capabilities of metabolic chronic disease teams in medical institutions.
(2) Assist at least 5 public medical institutions in the country to strengthen the hardware equipment needed for the prevention and treatment of metabolic chronic diseases.

3. Improve community people’s self-management capabilities required for the prevention and treatment of metabolic chronic diseases
(1) Capacity building of community health education personnel.
(2) Community health education promotion activities.
(3) Improve people’s management of their own metabolic conditions.

Completed the visit of three policy officials to Taiwan.
Completed the training of 4 medical seed trainees in Taiwan.
Completed the training of one community seed trainee in Taiwan.
Completed 1 health education training class and trained 22 people.
