Decades of economic growth and poverty reduction efforts in the peoples republic of china (prc) have been accompanied by improved living standards and health outcomes. basic health indicators such as maternal and infant mortality have shown drastic improvements, along with a decreasing incidence of infectious diseases. there has been a growing prevalence of noncommunicable diseases (ncds), heavily influenced by lifestyle-related factors such as poor diet, tobacco use, and excessive alcohol consumption as well as social isolation, contributing to growing incidence of mental health conditions, especially among the older persons. the prc has undergone a rapid demographic transition and is a moderately aged society, on its way to become a severely aged society by 2035. in 2020, it was estimated that the population over 60 years accounted for 18.7% of the total population it is expected that by 2050, the population above 60 years will reach 35%, in line with current trends of declining birth rates and reduced fertility rates.
the coronavirus disease (covid-19) pandemic highlighted major deficiencies in public health surveillance and management, and emphasized the importance of disease control as a regional public good. slow detection, response and reporting, and lack of coordination between agencies, highlights the health systems challenge to detect and respond to pandemics and public health events, and disproportionately affected older persons and the immunocompromised. lessons from the covid-19 pandemic indicate a vital need for more resources at the community and primary health care (phc) levels to detect and manage outbreaks, and improve the capacity of public health and primary health care institutions to coordinate responses.
located in the northwest prc, as of 2021, shaanxi has a total population of 39.54 million. the population is aging quickly-the proportion of people aged 65 years and older increased from 8.71% to 13.99% from 2011 to 2021. the aging population poses particular challenges in shaanxi due to poor delivery of integrated health and elderly care, especially in rural areas where populations are more widely spread. further, while data integration is critical for health and elderly care integration and quality phc, many issues remain. telemedicine and treatment at phcs are also unexplored, though it can improve access to health services in rural areas, which has significant time saving impact for women. capacity of phc services is also weak for ncd management, and disease detection and response. failure to address these issues is a bottleneck for improving health outcomes and disease control efforts, which has a far-reaching replicative impact for other provinces and has regional benefits since health is a global public good.
due to perceived poor quality, people commonly bypass phc facilities to obtain care in secondary and tertiary facilities which contributes to inefficiencies in the health system. this is further rei
